Program Year 2019 (March 1, 2019 through February 29, 2020)
Lucas County offers three medical plan options for eligible employees and their eligible dependents for the 2018/2019 program year: Paramount Patient Center Medical Home Plan (PCMH), a 90/10 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay. Employees in the PCMH plan must meet with their participating physician, complete a physical exam and enter into a "Partnership Agreement" with their physician. Paramount Health Care (PHC), a 75/25 HMO plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay; and the Lucas County Health Plan through the Frontpath Network, a 70/30 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay. The benefit levels through each of these three plans are not the same. *NEW* Effective March 1, 2018, employees who are enrolled in any one of the medical plans, will have a payroll deduction of $25.00 per single contract per month and $50.00 per family contract per month. The payroll deductions will be taken pre-tax. This means the actual amount taken out of your paycheck will be $11.54 per pay for a single plan and $23.08 per pay for a family plan. The payroll deduction will remain in effect for a period of two years-March 1, 2018 through February 29, 2020.
